Is a physician's order required to start? No. Our Nurse Care Management Program is private pay, which means your family can start without a physician's order or insurance approval. You reach out, we assess, and we get to work.
How is nurse care management different from having a caregiver? A caregiver provides hands-on daily support. Our nurse care managers provide clinical oversight — assessing, coordinating, communicating, and staying ahead of changes in condition. The two work well together, and many families have both in place.
What happens after the initial assessment? After the initial assessment, your nurse care manager develops a personalized care plan and begins active oversight. From there they stay involved on an ongoing basis — monitoring for changes, coordinating with providers, and keeping your family informed every step of the way.
